Easy way: make the switches operate a piston and have the piston push a sand block around until it meets a repeater near the door. Once it goes imbetween the repeater and another redstone it will send the signal through it to the redstone therefore opening the door. Voila.

What I did is I used RS NOR latches for buttons for the code. Works much better

MChelper40 made a much easier one, I made one with 100 levers

You can make locks smaller than this.

the problem with using levers with combo locks is that you can't reset them from inside your house! i find buttons a lot more efficeint!
